Clients must, prior to the commencement of the first session (no-cost assessment), complete, sign, and return the "Nurse Coaching Agreement" and "Confidentiality and HIPAA Consent" documents, which may be downloaded as a single PDF file.  Also, plan to download and complete the client information form, bringing it to your first appointment; or, you can obtain and complete this simple form when you arrive.
